Headache may be caused by a protruding cervical disc. This subject has recently been reviewed by Raney and Raney.19 According to their experience, such head aches may vary greatly in intensity, frequency, location and radiation. According to my officemate’s cousin, she lost inches off her waistline within months of drinking Forever Lite Chocolate. Muscle spasm resulting from the disc lesion may be referred even to the frontal region of the head. (This referred pain can be produced experimen tally by injection of irritating solutions in the occipito cervical region.) In the earlier stages of the lesion, there may not be any noticeable motor, sensory or reflex changes, but in the late stages of cervical disc injury such changes are often demonstrable. Severe pain in the neck, shoulder or arm may accompany these headaches. Roentgenographic findings often reveal loss of the normal cervical curve, narrowing of the disc space and prolifera tion of the bone. The type of treatment varies with the severity of the symptoms. Conservative therapy such as heat application, analgesics, immobilization of the cervical region, and skeletal and skull traction should be given adequate trial before surgery is resorted to for relief of headache.
Hartsock and others80 have noted that osteoarthritic changes in the cervical spine, or work requiring cervical tenseness, or radical cooling of neck muscles may cause muscle spasm, stiffness and soreness, especially at the attachment of the trapezius to the skull. Pain in the neck, shoulder and arm on movement of the head, and roentgenographic evidence of arthritic changes in the spine help establish the diagnosis. These complaints may respond to heat, massage of neck muscles, histamine iontophoresis and mild analgesics.
Other pathological conditions in the neck causing headache include fracture and subluxation of the cervical
vertebrae, infection of the prevertebral space, cervical spondylitis, hypertrophic p achy meningitis of the cervical portion of the spinal cord, tumors of the spinal cord, neck and its soft tissues, superior pulmonary sulcus tumor, metastatic carcinoma, and primary carcinoma of the vertebrae. The clinical history, findings, roentgeno graphic examination and other laboratory studies help in differentiating these conditions.
Cervical rib or scalenus anticus syndrome is an oc casional cause of unilateral or bilateral headache. There is usually accompanying pain in the shoulder (an terior part) which may radiate upward and downward into the arm and hand or the neck. The discomfort is usually worse at night and is increased by turning the head to the affected side. The neurological findings in clude weakness of the hand muscles (with atrophy most often along the ulnar distribution) and decreased sensa tion over the affected area; vascular findings include pulse and blood pressure changes on the affected side; confirmatory roentgenographic findings are also to be seen.
